本平台所載的保健品資訊（包括但不限於千和雲芝、千和蟲草等產品）均由供應商提供，僅供參考。產品功效因人而異，使用前請諮詢專業醫療人員意見。本平台不對任何產品效果作出保證。

本平台提供的體檢服務計劃及中醫服務資訊，均為第三方合作機構（包括但不限於體檢中心、中醫診所）提供的服務。本平台作為資訊平台，不直接提供醫療服務，亦不對第三方服務質素、結果或安全性作出任何保證。
閣下透過本平台預約的任何體檢或中醫服務，其合約關係直接存在於閣下與服務提供方之間。就因該等服務引致的任何損失、申索、損害、費用或開支，本平台概不負責。
4. 網上預約系統
透過網站/應用程式操作的網上預約系統只為協助閣下向合作機構/中心作預約醫療服務申請之用。提交網上預約申請並不保證閣下之預約成功。本平台將處理閣下的預約申請，並以電郵、電話、短訊(SMS)或Whatsapp就預約結果聯絡閣下。
本平台對透過此網上預約系統所作的預約申請而未能成功預約而引致的任何損失或後果，概不負責。透過網站/應用程式提交網上預約申請，即表示閣下已同意受遵守及受本聲明所約束。
5. 電子商務免責聲明
本平台提供的保健品銷售服務，產品由第三方供應商提供。本平台已盡合理努力確保產品資料準確，但不保證產品資料絕對準確無誤。產品圖片僅供參考，一切以實物為準。
閣下透過本平台購買的任何商品，其買賣合約直接存在於閣下與供應商之間。本平台不對產品質量、適用性或安全性作出任何明示或默示保證。

為確認閣下之身份、向閣下提供健康資訊、產品銷售、體檢及中醫服務預約（統稱「該等服務」）及為其他目的，本平台可能從閣下及/或已獲閣下授權的第三方收集有關閣下的個人資料，並持有該等個人資料，當中包括，但不限於，下列各項：
閣下個人的資料（包括閣下的姓名、性別、出生日期、身份證明文件種類和號碼、國籍、婚姻狀況）；
閣下的通訊資料（包括通訊地址、電話號碼、電郵地址及通訊語言）；
閣下之緊急聯絡人及/或親屬個人的資料及聯絡資料；
閣下的信用卡或扣賬卡帳戶及帳單資料（包括持卡人姓名、信用卡或扣賬卡號碼及有效日期）；
閣下的健康相關資料（包括就體檢或中醫服務預約時提供的健康狀況、病歷、藥物紀錄）；

1.2
當閣下使用本平台的網站時，本平台可能收集和儲存有關閣下在本平台網站上的瀏覽活動及使用網站的偏好的資料，以便進行統計和內部分析。例如，本平台會統計進入本平台網站個別網頁的次數/訪客人數及收集網站使用者的一般使用模式，藉以量度本平台網站的成效及改善本平台網站的可用性和本平台網站提供的服務。本平台的伺服器會記錄使用者瀏覽網站時的IP（網絡協定）位址、進入/瀏覽的日期和時間、所瀏覽的網頁和下載的文件檔案、曾瀏覽的網站以及使用的瀏覽器類型。此類資料均是匿名的，並且不包含姓名或地址或任何能夠讓任何人與閣下聯絡的資料。

當本平台從閣下收集個人資料時，本平台將以適當的格式及方式向閣下提供個人資料收集聲明（「個人資料收集聲明」），以向閣下指明收集閣下的個人資料的目的，以及本平台可能向那類型的第三方披露閣下的個人資料。

除以上所述，在未獲得閣下的事先同意的情況下，本平台將不會向任何第三方轉移或披露閣下的個人資料。
4. 直接促銷
4.1
根據《個人資料（私隱）條例》，本平台不會在未獲得閣下的事先同意的情況下，為直接促銷之目的使用閣下的個人資料。
4.2
如閣下已經給予同意而其後並無撤回該同意，本平台可使用閣下的個人資料，藉以透過不同的通訊方法（如短訊及電郵）向閣下送交有關本平台/關聯機構所提供的健康服務的宣傳和直接促銷資訊。
4.3
如閣下想本平台停止在直接促銷中使用閣下的個人資料，閣下可隨時致電 65249675，或透過本平台不時公佈的該等其他途徑發送有關要求。
5. 個人資料的保存
本平台不會把個人資料保存至超過達致原來收集以作使用，或予以使用，的目的（包括任何直接有關的目的）之所需時間。根據本平台的個人資料保存政策，不同的保存期將適用於本平台所收集和持有之不同類型的個人資料。
6. 個人資料的準確性和安全性
6.1
本平台設有適當的程序，藉以在本平台使用個人資料於收集該等個人資料的目的時，在合理的切實可行範圍內保持該等個人資料的準確性、完整性和相關性。

Any health check-up or Chinese medicine service booked by you through this Platform creates a contractual relationship directly between you and the service provider. This Platform shall not be liable for any loss, claim, damage, cost or expense arising from such services.
4. Online Booking System
The online booking system operated through the website/App is designed solely to assist you in submitting medical service appointment requests to partner institutions/centres. Submitting an online appointment request does not guarantee that your appointment will be successful. This Platform will process your appointment request and contact you by email, telephone, SMS or WhatsApp regarding the appointment result.
This Platform shall not be liable for any loss or consequences arising from unsuccessful appointments made through this online booking system. By submitting an online appointment request through the website/App, you agree to comply with and be bound by this statement.
5. E-Commerce Disclaimer
The health product sales services provided by this Platform are supplied by third-party suppliers. This Platform has made reasonable efforts to ensure product information is accurate, but does not guarantee that product information is absolutely accurate. Your personal information (including your name, gender, date of birth, type and number of identification document, nationality, marital status);
Your contact information (including correspondence address, telephone number, email address and correspondence language);
Your emergency contact and/or relatives’ personal information and contact information;
Your credit card or debit card account and billing information (including cardholder name, credit card or debit card number and expiry date);
Your health-related information (including health conditions, medical history and medication records provided when booking health check-up or Chinese medicine services);
Your purchase records and product preferences.
1.2
When you use this Platform’s website, this Platform may collect and store information about your browsing activities and preferences on this Platform’s website for statistical and internal analysis purposes. For example, this Platform counts the number of visits/visitors to individual pages of this Platform’s website and collects general usage patterns of website users, in order to measure the effectiveness of this Platform’s website and improve its usability and the services provided by this Platform’s website. This Platform’s servers record the IP (Internet Protocol) address, date and time of entry/browsing, pages viewed and files downloaded, websites visited and browser type used by users when browsing the website. Such data is anonymous and does not contain names or addresses or any information that would enable anyone to contact you.

This Platform does not attempt to identify the identity of users of this Platform’s website or their respective browsing activities from the data referred to in section 1.2, except in rare circumstances where law enforcement agencies may exercise a search warrant to inspect this Platform’s server records for investigation purposes. This Platform will keep your personal data confidential, but may transfer or disclose your personal data to this Platform’s staff and to the third parties specified in the relevant PICS (such as your attending doctor/healthcare professional, your insurer, Affiliated Institutions, this Platform’s agents and service providers and such other persons as permitted by law to whom disclosure may be made) for the purposes specified in the relevant PICS. This Platform will also disclose your personal data as required by any Law.
3.2
Except as described above, this Platform will not transfer or disclose your personal data to any third party without your prior consent.
4. Direct Marketing
4.1
In accordance with the Personal Data (Privacy) Ordinance, this Platform will not use your personal data for direct marketing purposes without your prior consent.
4.2
If you have given consent and have not subsequently withdrawn it, this Platform may use your personal data to send you promotional and direct marketing information regarding health services provided by this Platform/Affiliated Institutions through various communication methods (such as SMS and email).
4.3
If you wish this Platform to cease using your personal data for direct marketing purposes, you may call 65249675 at any time, or send such request through such other channels as may be announced by this Platform from time to time.
5. Retention of Personal Data
This Platform will not retain personal data for longer than is necessary to fulfill the purpose for which it was originally collected or used, including any directly related purpose. In accordance with this Platform’s personal data retention policy, different retention periods apply to different types of personal data collected and held by this Platform.
